Travel Permission & Rooming Policy
Parents or legal guardians must provide written permission for their child to travel and participate in any 919 Elite Football Club event if they will not be accompanying them. This permission must be submitted in the form of an official 919 Elite Travel Authorization Form or a signed note delivered directly to the player’s team coach prior to departure.

Please note: Players are strictly prohibited from rooming with any 919 Elite coach, staff member, or volunteer under any circumstances. All travel and lodging arrangements must comply with our club’s safety and supervision standards to ensure the well-being of every athlete.

Failure to comply with these policies may result in the athlete being ineligible to travel or participate in the event

In case of an emergency, I authorize the coaches and chaperones to seek medical attention for my child if necessary. Please note any medical conditions, allergies, or special needs. I understand that I will be contacted in the event of an emergency and will be responsible for any medical costs incurred.

I, the undersigned, am the legal parent or guardian of the child named above. I give permission for my child to participate in the above-mentioned event, and to travel with the team, coaches, and designated chaperones during the weekend trip. I understand that the coaches and chaperones will provide supervision and ensure my child's safety during this event. However, I acknowledge that there may be inherent risks involved in participating in sports-related activities and travel. I hereby release 919 Elite Football Club, its coaches, chaperones, and affiliates from any liability for injury or accidents that may occur during the event or travel
