Participation Release of Liability Waiver and Release. Participating in activities carries an inherent risk of injury, even if the greatest care is exercised. Accordingly, Participant(s) may injure himself/herself/themselves while attending or using 919 Elite Football facility or observing or participating in any of 919 Elite Football activities and/or programs, whether at 919 Elite Football or at a 919 Elite Football facility or site. Participant(s), therefore, assume(s) all risk of personal injury, death, property loss, theft, damage or destruction or other damages that may arise out of or in any manner relate to attending or 919 Elite Football facility or observing or participating in any of 919 Elite Football ’s programs or Activities, whether at 919 Elite Football or a non-919 Elite Football facility or site. By assuming these risks, Participant(s) waive(s) and release(s) any and all claims Participant(s) may have or may assert whether individually or jointly 919 Elite Football, including, but not limited to, 919 Elite Football’s members, officers, directors, managers, volunteers, coaches, employees, agents, facilities, sponsors, and representatives (collectively, “919 Elite Football Group”) for any harm such as personal injuries, death, property loss, theft, damage or destruction or other damages connected to, arising out of, or in any manner relating to any of the aforesaid risks. Participant(s) release(s) 919 Elite Football Group from any and all claims, damages, demands, rights of action, causes of action and liabilities, present or future, known or unknown, anticipated or unanticipated, resulting from or arising out of Participant(s) attendance at or use of 919 Elite Football’s facility or Participant(s) participation in any of 919 Elite Football’s Activities or programs, whether at 919 Elite Football or at a non-919 Elite Football facility or site, including, without limitation, those arising from 919 Elite Football’s negligence or that of 919 Elite Football Group. Participant(s) also release(s) 919 Elite Football Group from all liability relating to loss, theft, damage, or destruction to personal property, including, without limitation, automobiles, and locker contents. IN WITNESS WHEREOF, the parties have executed this Agreement, effective as of the date set forth below. Additionally, I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video tape without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording. I also understand that this material may be used in diverse educational settings within an unrestricted geographic area.
